Welcome to the Forms Portal!
Most forms are done "on-line", either on this website, the RankOne website, or redirection to Presto.
The Physical form is a "paper" form. Please print it out, fill and sign it, and remember to bring it to Registration.

There are MULTIPLE forms on the RankOne website for Band & Guard students.
A parent must complete the forms with the students. click the section title to go to the RankOne website. The RankOne website is scheduled to go live this year on Monday, March 24th, 2025. Here are the forms that are on RankOne:

Paper Form
All students must have a physical on file BEFORE August 1st to be eligible to participate in the physical aspects of marching band.
Get this completed and signed by your physician OR attend one of the drives hosted by our Frenship campuses. You can email completed forms to mlessing@frenship.us or upload it directly to RankOne. Click on the section title "Physical Form" to download the blank paper form, or you can download the same form from the Rank One website.

All personal student information will be recorded in Presto Assistant. Please note: parents and students have separate accounts.
Please routinely check your email for communication through Presto. Complete the FISD Instrument Loan Agreement form, if you are renting your instrument from Frenship ISD.
